TENECTASE® is indicated in thrombolytic treatment of acute ischemic stroke (AIS) within 4.5 hours of stroke initiation. TNK-t-PA (tenecteplase) is a modified version of natural tissue plasminogen activator produced by recombinant DNA technology in Chinese Hamster Ovary cells. TNK-t-PA is a 527 amino acid glycoprotein developed by substituting threonine 103 with asparagine and asparagine 117 with glutamine within the kringle 1 domain, and a tetra-alanine substitution replacing lysine, histidine, and two arginines at amino acids 296–299 in the protease domain of the mature polypeptide of native tPA. Sterile lyophilized powder of TNK-t-PA that is white to off-white in appearance must be reconstituted with Sterile Water for Injection (SWFI). Each vial of tenecteplase contains 20 mg of TNK-t-PA for single bolus intravenous (IV) administration.
